Its the endless number of revisions to get it just right.
From my experience it is not the writing of the document that takes the time but rather the review process and endless corrections in an attempt to get it one hundred percent right. That is why it is best not to have the document in the first place.
It is better to deliver 80% of the business value for 20% of the effort than to attempt to deliver 100% of the business value for 100% of the effort.
